An offer to come over and babysit one evening while the parents go out and have a grown up meal.
Beyond that, my throw-down shower gifts are generally a baby sling (in a colour and style that dad would feel comfortable using as well), and anything from Baby Bees -- they're stuff smells amazing, and ad pretty eco-friendly.

I bet she still has oodles of things from her first baby. And what she would really like is a manicure and pedicure.

If you can swing it, I second the babysitting evening! Sometimes the best gift is just a bit of a break :-)
Check out gdiapers.com, they have really cute designs, are environmentally friendly, and flushable (no more stinking pails!) I haven't tried them, but they seem like a great idea.

The last couple baby showers I've been to, I've given self-care goodies for the mom. Tummy oil, massage bars, foot rub stuff, with a suggestion that Dad get involved with the gift, too. Mom's have been appreciative, but I haven't heard back from any of the dads. :)
